Dosage

The brand Opsynvi is available in the strength of 10 mg/40 mg film coated tablets.

Usage

Opsynvi (macitentan, tadalafil) is used in adults (patients above 18 years of age) to treat certain types of a condition called pulmonary arterial hypertension (PAH), which is high blood pressure in the blood vessels leading to your lungs. Opsynvi can be taken on its own, or with other PAH medications as prescribed by your doctor. Opsynvi is a tablet that contains 2 different medicines called macitentan and tadalafil. The two medicines work together to widen the blood vessels leading to your lungs, which makes it easier for your heart to pump blood through them. This lowers high blood pressure in your lungs and lets your heart pump blood better. Opsynvi may lower the chance of your disease getting worse. Always take this medicine exactly as your doctor has told you. Do not stop taking Opsynvi unless your doctor tells you to. Swallow tablets whole, do not break, crush or chew the tablets. Opsynvi can be taken with or without food. Try to take Opsynvi at the same time each day.

Side Effects and Precautions

The common side effects of Opsynvi are back pain; constipation; diarrhea; eye pain; fatigue; flu (influenza); flushing of the face; headache; indigestion; muscle pain, spasms or stiffness; nausea or vomiting; pain in the arms or legs; rash; sore throat (pharyngitis); stomach pain; stuffy or congested nose (nasopharyngitis); upset stomach or heartburn.

Stop using Opsynvi and contact your doctor right away if you have any of the serious side effects such as:
• Anemia (decreased number of red blood cells): fatigue, loss of energy, irregular heartbeats, pale complexion, shortness of breath, weakness;
• Angina (chest pain): discomfort in the shoulder, arm, back, throat, jaw or teeth, shortness of breath, pain or pressure in the chest;
• Bronchitis (irritation of the airways): coughing, mucus production, fatigue, shortness of breath, slight fever and chills, chest discomfort;
• Edema: unusual swelling of the arms, hands, legs, feet and ankles, face or airway passages;
• Hypotension (low blood pressure): dizziness, fainting, light-headedness, blurred vision, nausea, vomiting, fatigue (may occur when you go from lying or sitting to standing up);
• Urinary tract infection (infection in urinary system including kidneys, ureters, bladder and urethra): pain or burning sensation while urinating, frequent urination, blood in urine, pain in the pelvis, strong smelling urine, cloudy urine.

Warnings and Interactions

Do not use Opsynvi if you:
• Are pregnant, think you may be pregnant, plan to become pregnant, or could become pregnant; Opsynvi can cause serious birth defects if taken during pregnancy;
• Are breastfeeding, or plan to breastfeed;
• Are taking any medicines that contain nitrates in any form (oral, under the tongue, skin patch, or by inhalation);
• Are taking any medicines that are called “guanylate cyclase stimulators”;
• Have ever had a type of eye disease called Non-Arteritic Anterior Ischaemic Optic Neuropathy (NAION), which causes a sudden decrease or loss of vision in one or both eyes;
• Have kidney problems. Opsynvi may lead to a reduction in blood pressure and decrease in haemoglobin in patients with kidney problems.

To help avoid side effects and ensure proper use, talk to your healthcare professional before you take Opsynvi about any health conditions or problems you may have, including if you:
• Are at risk of developing an eye disorder such as retinitis pigmentosa (a rare genetic eye disease);
• Have Pulmonary Veno-Occlusive Disease, a condition where your blood vessels are blocked;
• Have heart disease or previously had a heart attack;
• Have a condition called aortic stenosis (narrowing of the aortic heart valve);
• Have had a stroke;
• Have low blood pressure or uncontrolled high blood pressure;
• Have liver problems;
• Have kidney problems or are on dialysis;
• Have sickle cell anemia (an abnormality of red blood cells), multiple myeloma (cancer of the bone marrow), or leukemia (cancer of the blood cells);
• Have a deformed penis or are at risk of developing a condition called “priapism”;
• Have one of the rare hereditary diseases, because Opsynvi contains lactose: galactose intolerance; lapp lactase deficiency; glucose-galactose malabsorption.

If you feel chest pain after taking Opsynvi, do not take nitroglycerin or nitrates. Instead, get immediate medical help. Some patients using medicines like Opsynvi have experienced a sudden decrease or loss of hearing or a sudden loss of vision in one or both eyes. Get immediate medical help if either of these happen to you. Opsynvi may harm your unborn baby. You should not become pregnant while taking Opsynvi and for at least 1 month after stopping your treatment. Your doctor may recommend that you take a pregnancy test every month during your treatment to allow the early detection of pregnancy. Do not have unprotected sex. Tell your doctor right away if you have unprotected sex or if you think your birth control has failed. If you become pregnant, tell your healthcare professional right away. Stop taking Opsynvi. Some men have experienced a decrease in sperm cell count when taking medicines similar to Opsynvi. Speak with your doctor if you plan on fathering a child. Give yourself time after taking Opsynvi to see how you feel before driving or using machinery. Some patients taking macitentan were found to have abnormal liver function values (increase in liver enzymes) and some patients developed anemia (reduction in red blood cells). Your doctor will do regular blood tests to assess any changes in your liver function and hemoglobin level.

Tell your healthcare professional about all the medicines you take, including any drugs, vitamins, minerals, natural supplements or alternative medicines. The following may interact with Opsynvi:
• Alcohol;
• Amiodarone (medicine used to control heartbeat);
• Alpha blockers such as doxazosin (medicines used to treat high blood pressure);
• Bosentan (another treatment for pulmonary arterial hypertension);
• Cyclosporine (used to prevent organ rejection after transplant);
Diltiazem, verapamil (used to treat high blood pressure of specific heart problems);
• Grapefruit juice;
• Nefazodone (used to treat depression);
• Phenytoin, carbamazepine, phenobarbital (medicines used to treat seizures);
• Ritonavir, saquinavir (used to treat hiv infections);
• Rifampicin, clarithromycin, ciprofloxacin, erythromycin (antibiotics used to treat infection);
• Ketoconazole, itraconazole, fluconazole, miconazole, voriconazole (medicines used against fungal infections);
• Theophylline (medicine used to treat lung disease);
• Tablets for erectile dysfunction such as tadalafil.
