Usage

Xeljanz (Tofacitinib) is a prescription medication used to treat certain types of arthritis such as psoriatic arthritis, rheumatoid arthritis, polyarticular course juvenile idiopathic arthritis-pcJIA. This medication may also be used to treat moderate to severe ulcerative colitis in adults who cannot use certain medications, or after other treatments have failed. Xeljanz is used in adults who have tried Methotrexate and other medications without success in treatment of symptoms. Tofacitinib belongs to a class of medications known as Janus kinase inhibitors or JAK inhibitors.

Side Effects and Precautions

The common sides of Xeljanz may include abnormal blood tests, cold symptoms, diarrhea, fever, headache, increased blood pressure, nausea or vomiting, skin rash or shingles.

Some people taking high dosages of Xeljanz have developed serious or fatal blood clots in the lungs. Stop taking Xeljanz and seek emergency medical help if you have:
• Cough with pink or red mucus;
• Heavy sweating, clammy or blue-colored skin;
• Pain in your chest or back;
• Pain, swelling, or redness in an arm or a leg;
• Pain while breathing;
• Sudden shortness of breath

Call your doctor right away if you have:
• Low red blood cells;
• Signs of hepatitis;
• Signs of an infection;
• Signs of Perforation in your stomach or intestines;
• Signs of shingles

Warnings and Interactions

You should not use Xeljanz if you are allergic to Tofacitinib or if you have any kind of infection. Before you start taking this medication, tell your doctor about your medical history and if you are scheduled to receive any vaccine, or if you have ever had:
• A blood clot;
• A chronic infection;
• A stomach or intestinal problem such as diverticulitis;
• Diabetes;
• Heart problems (if you are over 50);
• HIV, or a weak immune system;
• Kidney disease or if you are on dialysis;
• Liver or lung disease

You should also tell your doctor if you have ever had tuberculosis or if anyone in your household has tuberculosis. Do not receive a "live" vaccine while using Xeljanz or you could develop a serious infection. Tell your doctor about all the medications you take including prescription, OTC, herbal supplements, and vitamins. There may be a drug interaction between Tofacitinib and other medications such as azathioprine, cyclosporine, and other drugs to treat arthritis or ulcerative colitis. Do not breastfeed while you are using Xeljanz and for at least 18 hours after your last dose of regular tablets and 36 hours after you take the XR tablets. Consult your doctor if you are pregnant. Xeljanz XR is not approved for use by anyone younger than 18 years old.
